Ways we can assist: lodging expenses, mortgage/rent assistance and gas reimbursement.
In order to apply, please see the requirements listed:
– Families must have a child with a congenital heart defect who are actively receiving care in the hospital
– Families main residence  must be within the Midwest states: Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, North Dakota, Ohio, South Dakota, and Wisconsin
– Must have a social worker contact and validation from the hospital your child is receiving care
– If you are requesting mortgage/rent or utility assistance, you must be able to provide a copy of monthly utility bill, mortgage/rent statement, or be able to provide gas receipts for mileage reimbursement
